This Social Work position involves an assignment in a Nursing Building. This employee functions as a member of a multidisciplinary treatment team and is involved in providing group treatment programs to the patients, as both a primary therapist and back up.
Social work responsibilities include the completion of Initial and Annual Assessments, the development of a Treatment Plan with a multidisciplinary team, and the provision of appropriate clinical services. The social worker serves as a liaison/therapist/resource to families and community agencies. Discharge planning is done with recognition of the special needs due to severe and pervasive mental illness, recurring medical conditions and physical limitations. The Social Worker must observe all reporting requirements of the Hospital, Social Work Department and accrediting agencies, such as Medicare, Medical Assistance.

Minimum Experience and Training Requirements
- Bachelor's or master's degree in social work or social welfare

Must be able to perform essential job functions
This position comes under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to PA State Police and FBI clearance.

A conditional offer of employment will require satisfactory completion a medical examination and drug screening test.
