Perry County Area Agency on Aging is actively looking to hire an Aging Care Manager 3. This vital role involves investigating allegations of abuse, neglect, exploitation, and abandonment, as well as creating tailored service plans and overseeing specialized services for the elderly population. In this position, you will be responsible for investigating claims of abuse and providing services aimed at identifying, addressing, or mitigating the risks associated with various forms of abuse. Your primary focus will be on ensuring the well-being of older adults, which encompasses evaluating their living conditions, financial situations, personal safety, and overall quality of life. You will interpret and apply relevant protective services legislation, regulations, policies, and operational procedures to guide your work. Additionally, you will assess the medical, social, and psychological needs of older adults, conducting thorough evaluations and entering data into a statewide database within specified deadlines. Collaboration with professional staff from other program areas will be essential as you discuss assessments, diagnoses, and treatment or support plans. You will also be tasked with developing, implementing, monitoring, and adjusting person-centered care plans tailored to the needs of older adults. Coordination with other agencies will be necessary to facilitate referrals for additional services, ensuring that older adults can access the resources they require. Furthermore, you may be called upon to testify in court regarding various aspects of the cases you are assigned, as needed.
Work Schedule and Additional Information:- Full-time employment
- Work hours are 8:00 AM to 4:00 PM, Monday - Thursday, with 60-minute lunch.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

- Hiring preference for this vacancy may be given to candidates who live within Perry County. If no eligible candidates who live within Perry County apply for this position, candidates who reside in other counties may be considered.

- This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
