VAPHS - HJ Heinz Community Living Center is seeking an experienced and highly skilled RN to be one of our
Resident Assessment Coordinators (RAC) who will provide oversight of the MDS 3.0 program.
The individual will function as the Coordinator of the RAI-MDS assessment process according to the CMS regulations and guidelines and Long-Term Care Institute Standards.
The Resident Assessment Coordinator (RAC) is responsible for initiating the appropriate MDS 3.0 assessments, signing, sealing and transmitting the assessments in a timely manner 100% of the time to avoid losing financial reimbursement. He/she will coordinate the completion of MDS 3.0 in an accurate and timely manner through scheduling of the MDS 3.0 assessments; providing staff education and training for all disciplines involved with the completion of MDS 3.0 and participating in performance improvement initiatives as they pertain to MDS 3.0.
The RAC will be a subject matter expert for the Resource Utilization Group (RUG scores), Case Mix index and the Quality Measures/SAIL Data as they relate to the completion of the MDS 3.0.
The RAC is responsible for keeping current on all new change notices and procedures for the MDS program by participating in National & VISN MDS 3.0 program calls.
The position will have collateral duties such as providing coverage for the admission coordinator when needed and collect blood specimens from Central Venous Lines, ex: PICC lines and access med ports and will perform other administrative or clinical duties as assigned.
The RAC will report to the Nurse Manager of Geriatrics and Extended Care who reports directly to the VP of Geriatrics and Extended Care.Minimum Experience:
Basic knowledge of the RAI/MDS 3.0 process: Assessments, Care Plans, 802s, QMs and SAIL Data.
Initiates or actively participates on interdisciplinary committees or groups assuring survey readiness of the RAI-MDS process and compliance in adherence to VISN/VACO regulations and CMS guidelines.
Minimum of 2-3-year recent long-term care nursing experience is preferred.
Previous experience with Rehabilitation and skilled nursing Care patients preferred.
Ability to work in a self-directed manner, collaborating and coordinating with health care professionals and the interdisciplinary team.
Basic knowledge of and functional in basic computer operations and proficiency in keyboarding, Excel, Word, etc.
Must be self-directed and comfortable working independently, but able to function effectively as a team member.
Work Schedule: Monday - Friday 7:30-4:00 or 8:00-4:30

This is a bargaining unit position.
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment with a doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
